I've had a few problems with vanishing attachments. I think I've worked out what is occurring.
You can attach multiple images providing you don't preview the post. Post it as is without preview. When you preview, the extra attachments sometimes disappear from the attachment upload cache. I'm using a current Chrome browser with OS x High Sierra.
After so many months using [img] tags from a "forum illustrations image cache" I have on my website, I've got used to it and I think I'll just keep doing that.
Among the advantages there are no size limitations to the displayed image provided your website has no limitations. My website has 500GB of web server space and no limits on file size or image size.

I get a report that shows all the 'attempted' image posts that didn't end up in a post / tread. The list has been long but I assumed it was because of the 'gallery' problem we had.

Another solution might be to write your post, preview, then attach all images in a batch at the final step and post immediately.
I like to preview my longer posts. Easier to see if any edits are required.
After we work this stuff out, maybe we can produce a sticky or a help article so that every poster doesn't need to work it out for themselves.
